Integrate Health (IH) is a non-profit with nearly 20 years of experience building innovative healthcare solutions with local communities and Ministries of Health (MoHs) to make quality primary health care accessible to all. In Togo, IH has demonstrated the effectiveness of a community-based health system strengthening programme called the Integrated Primary Care Program (IPCP), with published results showing a 30% decrease in infant mortality rates in communities where this approach has been implemented.
Following the success of IPCP in Togo, Integrate Health partnered with the Government of Guinea and Gavi to launch a US$ 6 million initiative in 2023, aimed at reaching zero-dose and under-immunised children while strengthening community health systems. Supported through contributions from private sector partners matched by Gavi, the programme combines community health worker support, clinical mentorship, strengthened health infrastructure and supply chains with digital innovations to improve access to care.
Since launch, the programme has reached more than 200,000 people, trained 430 community health workers, achieved a 97% catch-up rate among identified zero-dose and under-vaccinated children, referred more than 5,400 children and pregnant women for care, and exceeded vaccination performance targets.
A key innovation has been Kendeya, a digital community health platform that improves community-level record-keeping, decision support and patient follow-up. Following demonstrated results, Kendeya was incorporated into Guinea’s National Community Health Strategy 2026–2030, creating a pathway for national scale-up.
Building on these achievements, the partnership has been extended through 2030, supported by a performance-based funding framework from The Church of Jesus Christ of Latter-day Saints that could mobilise up to US$ 7.5 million over five years to sustain and expand the model. The extension positions the programme to further reduce the number of zero-dose children and strengthen integrated primary health care delivery in Guinea and beyond.
